Field Trips

Field Trip Permission Form

Extended School Trip Request/Planning

This request should be submitted six weeks before the trip date. Trips which are 50 miles away or more and/or overnight are required to have school board approval.

Complete and submit this form only once. If you have future changes to the trip details, e-mail the Transportation Department at transportation@haverfordsd.net. You will receive an e-mail confirmation.
