Field Trip Permission Form

• Self Carry/Administration of emergency medication (Physician’s Order)

• Physician’s Order for Prescription & Over-the-counter medication (Consent Form) 

Extended School Trip Request/Planning 

• This request should be submitted six weeks before the trip date. It is required to get board approval for trips of 50 miles or more or overnight.

• Complete and submit this form only once. If you have future changes to the trip details, e-mail the Transportation Department at transportation@haverfordsd.net. You will receive an e-mail confirmation.
