- Field Trip Parent/Guardian Permission Form
- Extended School Trip Request/Planning - Trips that are 50 miles away or more and/or overnight are required to have school board approval. These requests should be submitted at least six weeks before the trip date.

Transportation Field Trip Protocol
- Transportation Request Form - Complete and submit this form only once. If you have future changes to the trip details, e-mail the Transportation Department at transportation@haverfordsd.net. You will receive an email confirmation.
