This form is required for compliance underthe Pennsylvania Child Labor Law Act of 1915, P.L. 286, No. 177. Persons returning this form should complete all pages, Including additional pages or attach any additional information, which may be needed and applicable to the specific Law under which complianceis made. Student Hours of Employment: v Ages 14 & 15 (Vacation Employment Certificate): o Maximum of 3 hours on a school day (18 hours in a school week) o Maximum of 8 hours on a non-schoolday (40 hours in a non-school week) o May not work after 7pm or before 7am during the school term (may work until 9pm during the summer vacation) v Ages 16 & 17 (Transferable Working Permit): o Maximum of 8 hours on any given day (28 hours Monday through Friday plus an additional 8 hours on Saturday and 8hours on Sunday maximum 44 hours per week) o May not work after midnight Sunday through Thursday or before 6am any day (may work until 1am on a night preceding a non-school day) For more information, please call The School District of Haverford Township's Administration Office at (610) 853-5900 (Sheron ext. 7000).
Instructions For Attaining PA State Working Papers: If you live in the Haverford Township School District, you may obtain an application on-line or at the Oakmont Administration Officeat 50 East Eagle Road, Havertown, PA 19083 between the hours of 8:30-4:15pm, Monday through Friday. (Summer hours, 8:30-3:45pm). Please follow the instructions on filling out the APPLICATION for student.Section A. In order forworking papers to be issued, the applicant MUST have reached his/her 14th birthday. vStudents must complete areas in Section B. C. & D, If these areas are not filled out, working papers cannot be issued: Section B. Legal documentation showing the minor's birth date (i.e. birth certificate, driver's license/learner's permit, passport, baptismal certificate or student school I.D.). Section C. Hours and name of prospective employer must be provided. Section D. Physician office must signand date application indicating the minor is physically qualified foremployment. Important Notice: Any student hired by the Haverford Township School District must get a TB shot (Tuberculosis) as a requirement for employment. v When application is completed and returned,the student MUST be present to sign the worker’s permit card.
